Resumo: Introdução: O ensino médico vem passando por transformações nas últimas décadas. Objetivos educacionais tendem a se alterar com os avanços tecnológicos recentes, em especial na área de tecnologias de informação. Objetivo: Esta pesquisa aborda o exame do fundo de olho explorando e analisando as dificuldades dos estudantes de Medicina na execução desse componente do exame clínico e busca propor diretrizes para seu ensino na graduação médica. Métodos: Trata-se de uma pesquisa qualitativa com técnicas de observação direta e entrevistas com análise de conteúdo em uma população de estudantes do internato da Universidade do Estado do Pará (Uepa), na cidade de Marabá. Na avaliação de conteúdo utilizaram-se recursos do programa livre de análise de texto Iramuteq. Resultados: Dos 21 estudantes voluntários participantes da pesquisa, apenas dois relataram experiência anterior com oftalmoscópio direto (9,52%) e um aluno havia participado de campanha com uso de dispositivo portátil para registro da imagem do fundo de olho (4,8%). As atividades da pesquisa incluiram discussão de casos clínicos, realização de oftalmoscopias diretas em pacientes voluntários e análise de retinografias. Na análise dos textos correspondentes às entrevistas foram categorizadas quatro classes geradas pelo programa Iramuteq, realçando-se o valor da integração de teoria e prática no depoimento dos alunos. Conclusão: Programas de treinamento com integração de teoria e prática e valendo-se de princípios de aprendizagem significativa podem contribuir para prover competência ao estudante de Medicina para o exame de fundo de olho, adequando-se ao surgimento de novas tecnologias.


Abstract: Introduction: Medical education has undergone changes in recent decades. Educational objectives tend to change with recent technological advances, especially in the area of information technologies. Objective: This research addresses the examination of ocular fundus by exploring and analyzing the difficulties medical students encounter in relation to this component of the clinical examination and seeks to propose guidelines for its teaching in undergraduate medical training. Method: Qualitative research with direct observation techniques and interviews with content analysis with a population of students at the University of the State of Pará (UEPA), in the city of Marabá. Content analysis used resources from the free text analysis program Iramuteq. Results: Of the 21 volunteer students participating in the research, only two reported previous experience with direct ophthalmoscope (9.52%) and one student had participated in a campaign using a portable device to record the fundus image (4.8%). Research activities included discussion of clinical cases, performing direct ophthalmoscopies in volunteer patients and retinography analysis. In the analysis of the texts corresponding to the interviews, four classes generated by the Iramuteq program were categorized, highlighting the value of theoretical and practical integration in the students' testimony. Conclusion: Training programs with theoretical and practical integration using meaningful learning concepts can help qualify medical students in the ocular fundus exam, adapting to the emergence of new technologies.

ABSTRACT

Resumo Objetivo: o ensino da oftalmoscopia direta vem recebendo menos atenção na graduação, o que resulta em médicos sem confiança para realizá-la. Diante disso, foi analisada a autopercepção dos acadêmicos dos cursos de medicina de Mato Grosso sobre a oftalmoscopia direta e os principais fatores responsáveis pela insegurança em executá-la. Métodos: foi realizada a revisão bibliográfica nas bases Pubmed, Scielo e Biblioteca Virtual em Saúde, sendo construído um questionário aplicado, pelo método bola de neve e após parecer favorável do Comitê de Ética, a acadêmicos de medicina do Mato Grosso que estavam cursando regularmente o quinto ou sexto ano do curso. A análise estatística foi realizada no software SPSS 20, considerando intervalo de confiança de 95%. Resultados: verificou-se que a proporção de acadêmicos que tiveram aulas sobre a oftalmoscopia direta foi superior em universidades públicas (p < 0,001); que a maioria dos alunos não acredita que suas escolas forneçam as condições adequadas para o aprendizado do exame (p = 0,001); que 10,5% se consideram conhecedores da teoria e prática da oftalmoscopia direta (p = 0,004); que a falta de treinamento durante a graduação é o principal fator que dificulta o aprendizado do exame; e que isso resulta em falta de confiança em realizar o mesmo. Conclusão: a oftalmoscopia direta não tem recebido a devida importância durante a graduação em medicina no estado de Mato Grosso. Com tais resultados, pode-se tomar medidas que melhorem o ensino-aprendizado da técnica, o que posteriormente trará benefício para a população.


Abstract Objective: direct ophthalmoscopy teaching is receiving less attention during graduation, which results in doctors without confidence to perform the exam. Therefore, the self-perception of students from medical schools of Mato Grosso about the direct ophthalmoscopy and the main factors responsible for the insecurity on performing it was analyzed. Methods: a literature review was done using the databases Pubmed, Scielo and Biblioteca Virtual em Saúde, then a questionnaire was applied, through the snowball method and after favorable assent of the Ethics Committee, to medical students from Mato Grosso who were regularly enrolled in the fifth or sixth year of the course. Statistical analysis was performed on the SPSS 20 software, considering a confidence interval of 95%. Results: it was verified that the proportion of students that have had classes about direct ophthalmoscopy was superior in public schools (p < 0,001); that the majority of students does not believe that their schools provide adequate conditions for learning the exam (p = 0,001); that 10,5% considerate themselves knowledgeable about theory and practice regarding direct ophthalmoscopy (p = 0,004); that few training during graduation is the main factor that hinders learning the exam; and that this results in underconfidence at performing it. Conclusion: direct ophthalmoscopy is not receiving the deserved importance during medical graduation in the state of Mato Grosso. With these results, measures can be taken to improve the teaching-learning process of the technique, which can bring benefit to the population.

Resumo Objetivo: Analisar a prevalência de alterações na fundoscopia de pacientes portadores de Diabetes Mellitus (DM) e/ou Hipertensão Arterial Sistêmica (HAS) encaminhados ao Serviço de Referência de Oftalmologia, localizado em uma Unidade Básica de Saúde do município de Patos, Paraíba (PB). Metodos: Estudo de caráter descritivo, transversal e com abordagem quantitativa, que envolveu um total de 22 pacientes. Para a coleta de dados, foi utilizado um questionário sobre a saúde ocular. Além disso, os integrantes da pesquisa participaram do teste da acuidade visual de Snellen e acuidade visual para perto, da medida da pressão intra-ocular e do exame da oftalmoscopia direta. Os que obtiveram pior acuidade visual foram conduzidos para a realização de retinografia. Os dados foram avaliados através de análises estatísticas utilizando o Statistical Package for the Social Sciences - SPSS. Resultados: Do total de 22 pacientes, 11 foram submetidos ao exame de retinografia. Destes, 36,3 % eram apenas diabéticos; 27,4% eram apenas hipertensos e 36,3% eram diabéticos e hipertensos. As principais alterações encontradas foram retinopatia diabética, retinopatia hipertensiva, catarata, glaucoma, nevus de coroide, retinose pigmentar e estafiloma peripapilar. Conclusão: Percebe-se que DM e HAS tem grande impacto negativo sobre a saúde ocular. Para diminuir esse efeito nocivo é necessário que o exame de fundo de olho seja realizado anualmente com a finalidade de diagnosticar precocemente certas patologias e evitar complicações futuras, culminando em menores custos para o sistema de saúde e mais qualidade de vida para os pacientes.


Abstract Objective: To analyze the prevalence of alterations in funduscopy of patients with Diabetes Mellitus (DM) and/or Systemic Arterial Hypertension (SAH) referred to the Ophthalmology Reference Service, located in a Basic Health Unit of the city of Patos, Paraíba). Methods: A descriptive, cross-sectional study with a quantitative approach, which involving a total of 22 patients. For data collection, a questionnaire on ocular health was used. In addition, the research members participated in the Snellen visual acuity test and visual acuity near, intraocular pressure measurement and direct ophthalmoscopy examination. Those who obtained worse visual acuity were conducted for retinography. The data were evaluated through statistical analysis using the Statistical Package for the Social Sciences - SPSS. Results: Of the total of 22 patients, 11 were submitted to retinography. Of these, 36.3% were diabetics only; 27.4% were hypertensive only and 36.3% were diabetic and hypertensive. The main alterations found were diabetic retinopathy, hypertensive retinopathy, cataract, glaucoma, choroidal nevus, pigmentary retinitis and peripapillary staphyloma. Conclusion: It is noticed that DM and SAH have a great negative impact on ocular health. To reduce this harmful effect it is necessary that the fundus eye exam be performed annually for the purpose of early diagnosis of certain pathologies and avoid future complications, culminating in lower costs for the health system and more quality of life for patients.

ABSTRACT

RESUMEN El objetivo de este reporte de caso fue identificar las características de un nevo abultado conjuntival en una paciente femenina de 72 años de edad. Se muestran las pruebas complementarias realizadas para llegar al diagnóstico de melanoma conjuntival, que consiste en un tumor poco frecuente, el cual constituye el 2 por ciento de todos los tumores del ojo. Las células involucradas son los melanocitos del epitelio conjuntival. Un factor de riesgo para esta entidad puede ser la exposición a la luz ultravioleta; sin embargo, esto no está bien determinado. La mayoría de los casos se originan de una melanosis adquirida primaria (50-70 por ciento). La presentación más común es una lesión pigmentada, unilateral, elevada, gruesa, con vasos sanguíneos y áreas de melanosis a su alrededor. Su diagnóstico se realiza con la sospecha clínica que se confirma con el estudio histopatológico. El tratamiento en la actualidad es la excisión local, seguida de crioterapia, radioterapia o quimioterapia. La enfermedad matastásica ocurre en el 11 - 42 por ciento y en el 20 - 52 por ciento de los pacientes en un seguimiento de 5 y 10 años, respectivamente(AU)


ABSTRACT The objective of this case report is to describe the characteristics of a conjunctival bulging nevus in a 72-year-old female patient and the complementary tests that were performed to reach the diagnosis of conjunctival melanoma. Conjunctival melanoma is a rare tumor, which account for 2 percent of all eye tumors. The cells involved are the melanocytes of the conjunctival epithelium. A risk factor for this entity may be exposure to ultraviolet light. However, it has not been precisely described. Most cases originate from a primary acquired melanosis (50-70 percent). The most common presentation is a pigmented, unilateral, elevated, thick, lesion with blood vessels and areas of melanosis around it. Its diagnosis is made with the clinical suspicion confirmed with the histopathological study. Current treatment is local excision, followed by cryotherapy, radiotherapy, or chemotherapy. Metastatic disease occurs in 11-42 percent and 20-52 percent of patients in a 5-year and 10-year follow-up, respectively(AU)

RESUMEN El fondo de ojo es considerado por muchas especialidades médicas un proceder complementario. ¿Existe una correcta indicación del fondo de ojo de urgencia? Objetivo: Identificar los principales motivos de indicación del fondo de ojo urgente y su pertinencia. Métodos: Se realizó un estudio descriptivo transversal en el periodo comprendido desde el 29 de mayo hasta el 1ro. de diciembre del año 2018, en una muestra de 59 pacientes de un universo de 67 casos con indicación de fondo de ojo urgentes, a realizarse en la consulta del Cuerpo de Guardia de Oftalmología en la Unidad de Cuidados Intensivos Especiales y en salas intrahospitalarias del Hospital General Docente Provincial "Antonio Luaces Iraola", de la provincia de Ciego de Ávila, Cuba. Resultados: El 28,8 por ciento de los fondos de ojos fueron ordenados por Medicina Interna, seguido por Pediatría y Ginecobstetricia con 20,3 por ciento cada uno. El antecedente patológico personal ocular más frecuente fue la ametropía y el familar el glaucoma. El antecedente patológico personal sistémico más frecuente fue la hipertensión arterial. Las razones que motivaron su indicación urgente, en orden de frecuencia, fueron: el síndrome de hipertensión arterial en gestantes, el síndrome de cefalalgia, el traumatismo craneoencefálico en puérperas, el síndrome febril y el síndrome vertiginoso. Solo en 2 pacientes se visualizaron signos de sospecha de glaucoma, sin otras alteraciones a nivel de la papila óptica, y retina del polo posterior en los casos restantes. Conclusiones: Todos los fondos de ojos realizados fueron negativos para papiledema, signo que presumiblemente debe descartarse, en dependencia del motivo y la especialidad que lo indique con urgencia(AU)


ABSTRACT The fundus of the eye is considered, by many medical specialties, a complementary procedure. Is there a correct indication of the fundus of the eye as an emergency? Objective: To identify the main reasons for the indication of the fundus of the eye as an emergency and its relevance. Methods: A descriptive and cross-sectional study was carried out in the period from May 29 to December 1, 2018, with a sample of 59 patients from a study group of 67 cases with indication of urgent fundus of the eye to be carried out in the service of the Ophthalmology Emergency Room belonging to the Special Intensive Care Unit and in hospital wards of Professor Antonio Luaces Iraola General Hospital of Ciego de Ávila Province, Cuba. Results: 28.8 percent of the fundus of the eye were ordered by internal medicine, followed by pediatrics and gynecobstetrics, each accounting for 20.3 percent. The most frequent personal pathological ocular history was ametropia and the glaucoma family. The most frequent systemic personal pathological history was arterial hypertension. The reasons that motivated its urgent indication, in order of frequency, were the syndrome of hypertension in pregnant women, the headache syndrome, the traumatic brain injury in postpartum women, the febrile syndrome, and the vertiginous syndrome. Only two patients showed signs of suspected glaucoma, without other alterations at the level of the optic papilla, and retinal posterior pole in the remaining cases. Conclusions: All the fundus of eyes carried out were negative for papiledema, a sign that presumably must be discarded, depending on the reason and the specialty that indicates it with urgency(AU)

ABSTRACT

Resumo A distrofia macular anular concêntrica benigna (DMACB) é uma patologia retiniana rara e provavelmente subdiagnosticada em nosso meio, que se caracteriza por um defeito retiniano em bull's eye sem uso prévio de antimaláricos, associado à preservação relativa da acuidade visual. Devido à escassez de publicações sobre o tema, existem poucos dados referentes aos resultados dos exames complementares nesta patologia. No presente artigo, apresenta-se a descrição da autofluorescência em um caso clássico de DMACB, ainda inédita na literatura, podendo acrescentar achados importantes para auxiliar no diagnóstico e seguimento da doença.


Abstract The benign concentric annular macular dystrophy (BCAMD) is a very rare and probably underdiagnosed eye disease, characterized by a retinal fault in bull's eye pattern, without the association with antimalarial use, but related with good visual acuity. Since there aren't many publications about this condition, is hard to find data regarding the results of complementary examination. In this article, is presented the description of fundus autofluorescence in a classic BCAMD case, yet unpublished, and capable of helping the diagnosis and follow-up of this pathology.

ABSTRACT Purpose: To determine the effect of panretinal photocoagulation on optic disk topographic parameters in non-glaucomatous patients with proliferative diabetic retinopathy. Methods: This was a prospective, single-center, observational study. Thirty-eight eyes of 26 patients with diabetes underwent panretinal photocoagulation for proliferative diabetic retinopathy. Stereoscopic disk photographs and optic nerve head parameters were evaluated using the Zeiss fundus camera and the confocal scanning laser ophthalmoscope (Heidelberg Retinal Tomograph), respectively, at baseline and 12 months after the completion of panretinal photocoagulation. Results: Thirty-eight eyes of 26 patients (15 female) with a mean age of 53.7 (range 26-74) years were recruited. No significant difference was found between the stereo photography determined mean horizontal and vertical cup-to-disk ratio before and after panretinal photocoagulation treatment (p=0.461 and 0.839, respectively). The global values of the optic nerve head parameters analyzed with the HRT3 showed no significant change from baseline to 12 months, including the disk area, cup area, rim area, cup volume, rim volume, cup-to-disk area ratio, linear cup-to-disk ratio, mean cup depth, maximum cup depth, cup shape measure, height variation contour, mean retinal nerve fiber layer thickness, and cross-sectional area. Conclusion: Our results suggest that panretinal photocoagulation does not cause morphological optic disk changes in patients with diabetic proliferative retinopathy after 1 year of follow-up.


RESUMO Objetivo: Determinar o efeito da panfotocoagulação retiniana nos parâmetros topográficos do disco óptico em pacientes não glaucomatosos com retinopatia diabética proliferativa. Métodos: Este é um estudo observacional pros­pectivo e unicêntrico. Trinta e oito olhos de 26 pacientes dia­béticos foram submetidos à panfotocoagulação retiniana para retinopatia diabética proliferativa. As estereofotografias e os parâmetros do disco óptico foram avaliados usando o retinógrafo Visucam da Zeiss e o oftalmoscópio confocal de varredura a laser (Heidelberg Retinal Tomograph), respectivamente, no início e 12 meses após a conclusão da panfotocoagulação. Resultados: Trinta e oito olhos de 26 pacientes (15 mulheres) com média de idade de 53,7 anos (intervalo de 26-74) foram recrutados. Nenhuma diferença significativa foi encontrada entre a média horizontal e vertical para relação escavação/disco óptico determinadas pelas estereo­fotografias antes e após o tratamento com panfotocoagulação retiniana (p=0,461 e 0,839, respectivamente). Os valores globais dos parâmetros do disco óptico analisados com a tomografia de varredura a laser não mostraram nenhuma mudança significativa entre o início até os 12 meses, incluindo disk area, cup area, rim area, cup volume, rim volume, C/D area ratio, linear C/D ratio, mean cup depth, maximum cup depth, cup shape measure, height variation contour, mean retinal nerve fiber layer thickness e cross-sectional area. Conclusão: Nossos resultados sugerem que a panfotocoagulação retiniana não causa alterações morfológicas no disco óptico em pacientes com retinopatia diabética proliferativa após um ano de seguimento.

ABSTRACT

Las fosetas congénitas papilares son poco comunes. Se trata de una entidad caracterizada típicamente por depresiones papilares unilaterales, ovales, de color blanco grisáceo. Reportamos un caso con dicha entidad asociada al desprendimiento seroso tratado con fotocoagulación. Se realizó un estudio descriptivo a una paciente femenina de 14 años quien refirió pérdida de visión del ojo izquierdo de un mes de evolución. Se realizó fotocoagulación láser al borde yuxtapapilar temporal de la foseta, con evoluciones al mes y a los 3, 6, 9, 12 y 24 meses posteriores, mediante tomografía de coherencia óptica, mejor agudeza visual corregida y oftalmoscopia. Se logró progresiva disminución del desprendimiento macular y mejoría de la agudeza visual desde el primer mes de control. A los 9 meses se observó normal arquitectura macular en la tomografía de coherencia óptica y mejoría de la agudeza visual a 20/25 (95 VAR), con iguales resultados en los controles a los 12 y 24 meses. La terapéutica con fotocoagulación puede resultar eficiente, simple, mínimamente invasiva y económica para la maculopatía asociada a la foseta papilar(AU)


Congenital optic disc pits are an infrequent condition typically characterized by unilateral oval grayish-white papillary depressions. A case report is presented of this condition associated to serous detachment treated with photocoagulation. A descriptive study was conducted of a 14-year-old female patient who reported loss of vision in her left eye with a month of evolution. Laser photocoagulation was performed on the temporal juxtapapillary edge of the pit, with evolution checks at 3, 6, 9, 12 and 24 months by optical coherence tomography, best corrected visual acuity and ophthalmoscopy. Progressive macular detachment reduction and visual acuity improvement were observed since the first control. At 9 months optical coherence tomography showed a normal macular architecture and visual acuity improvement to 20/25 (95 VAR) with identical results at 12 and 24 months. Photocoagulation therapy may be efficient, simple, minimally invasive and economical for maculopathy associated to optic disc pits(AU)

ABSTRACT Purpose: To determine the relationship between anterior segment and optic nerve head (ONH) parameters. Methods: Two hundred healthy subjects were enrolled in this study. The par­ticipants underwent measurement of anterior segment parameters with a Pentacam Scheimpflug system and imaging of the optic disc with confocal scanning laser ophthalmoscopy. Pearson's correlation coefficient was calculated to assess the associations between ONH and anterior segment parameters. Multivariate regression analyses controlling for age and disc area as confounding factors were also conducted. Results: There were significant negative relationships between optic disc area and corneal thickness (p=0.03, r=-0.217) and volume (p=0.017, r=-0.239). Corneal refractive power was significantly correlated with cup area, rim area, rim volume, and cup/disc ratio (p<0.05 for all). An increase of 1 diopter in anterior corneal refractive power corresponded to a decrease of 0.022 in cup/disc ratio. Rim volume was negatively correlated with anterior chamber depth and anterior chamber volume (p<0.05 for both). A 1-mm3 increase in anterior chamber depth corresponded to a 0.154-mm3 decrease in rim volume. Mean and maximum cup depth measurements were not associated with any of the anterior segment parameters. Conclusions: In nonglaucomatous eyes, the dimensions and shape of the ONH may be related to anterior segment morphology. Therefore, it may be helpful to take anterior segment parameters into consideration in evaluating the OHN.


RESUMO Objetivo: Determinar a relação entre os parâmetros do segmento anterior e da cabeça do nervo óptico (CNO). Métodos: Duzentos indivíduos saudáveis foram incluídos neste estudo. Os participantes foram submetidos à medição dos parâmetros do segmento anterior com o sistema Pentacam Scheimpflug e à obtenção de imagens de disco óptico com oftalmoscopia a laser de varredura confocal. O coeficiente de correlação de Pearson foi calculado para avaliar as associações entre CNO e os parâmetros do segmento anterior. Realizaram-se também análises de regressão multivariada que controlam a idade e a área do disco como factores de confusão. Resultados: Houve uma relação negativa estatisticamente significativa entre a espessura da córnea, o volume da córnea e a área do disco óptico (p=0,03, r=-0,217 e p=0,017, r=-0,239, respectivamente). As medições do poder de refração da córnea mostraram correlações significativas com a área da escavação, a área da rima, o volume da rima (RV) e a relação escavação/disco (C/D). (P<0,05 para todos). Um aumento de dioptria no poder de refração corneana anterior corresponde a uma diminuição de 0,022 em C/D. O RV mostrou correlação negativa com a profundidade da câmara anterior (PCA) e volume da câmara anterior. (P<0,05 para ambos). O aumento de 1 mm3 na PCA corresponde a uma diminuição de 0,154 mm3 no RV. As médias e as medidas máximas de profundidade de rima não mostraram associação com nenhum dos parâmetros do segmento anterior. Conclusões: Em olhos não glaucomatosos, as dimensões e forma da CNO podem estar relacionadas com a morfologia do segmento anterior. Portanto, pode ser útil levar em consideração os parâmetros do segmento anterior na avaliação da CNO.

ABSTRACT

Desde el primer reporte de pliegues coroideos, en el año 1884, el conocimiento sobre su etiología, el mecanismo de producción y su manejo se ha incrementado notablemente. Las causas de los pliegues coroideos abarcan un extenso número de condiciones muy variadas; entre ellas se encuentran los idiopáticos, la hipertensión intracraneana, algunas drogas como el topiramato, las infiltraciones difusas de la coroides por linfomas e hiperplasia linfoide, la hipotonía por contracción de tejido fibrovascular, los tumores coroideos y orbitarios (intraconales y extraconales), entre otras. Se presenta una paciente femenina de 57 años de edad quien acudió por vez primera a la Consulta de Oftalmología en el año 2008 por disminución lenta y progresiva de la agudeza visual de ambos ojos. El examen de fondo de ojo mostró discos ópticos con bordes definidos, buena coloración y presencia de pliegues coroideos radiales al disco en polo posterior de ambos ojos. La refracción arrojó una hipermetropía que fue en aumento en las consultas de seguimiento hasta el presente sin variar el aspecto del fondo de ojo. Se concluye que el caso presenta síndrome de hipermetropía adquirida con pliegues coroideos(AU)


Since the first reported case of choroidal folds in 1884, the understanding of their mechanisms, aetiologies, and management has expanded signally. With ophthalmoscopy, choroidal folds can be appreciated by the light and dark bands observed deep to the retina. While choroidal folds are visible on ophthalmoscopic examination, they are more easily identified using fluorescein angiography. A-scan ultrasound may reveal a shortened axial length. Common B-scan ultrasonographic findings include thickening of the choroid or flattening of the posterior aspect of the globe. Causes of choroidal folds are many, among them are idiophatic, increased intracranial pressure, diffusely infiltrative conditions (lymphomas and benign lymphoid hiperplasia), hypotony, drug induced (an unusual complication of certain medications such as Topiramate), contraction of fibrovascular tissue, choroidal neoplasms and orbital mass lesions (intraconal and extraconal tumours). We report a 57 years old woman who came to ophthalmogy consultation first in 2008 complaning of slowly progressive loss of vision. Fundoscopy showed well defined optic disc with radial choroidal folds of the posterior pole in both eyes. Manifest refraction showed hyperopia that increased in the following visits until the present. Fundoscopy showed not changes. Definitive diagnosis was syndrome of acquired hyperopia with choroidal folds(AU)

ABSTRACT Purpose: In 2015, a twenty-fold increase in the prevalence of microcephaly in Brazil was reported, and the Ministry of Health associated this abnormal prevalence with the maternal-fetal Zika virus (ZIKV) transmission. Methods: We assessed the ophthalmological findings of ten mothers and their infants that had been clinically diagnosed with ZIKV-related microcephaly and presented ocular abnormalities, born from May to December 2015. Results: Seven mothers (70.0%) referred symptoms during pregnancy (malaise, rash and arthralgia), of which six (85.7%) were in the first trimester. At the time of exam, no ophthalmological abnormalities were identified in the mothers and they did not report ocular symptoms during pregnancy. Serology was negative in all infants for Toxoplasmosis, Rubella, Cytomegalovirus, Syphilis and Human Immunodeficiency Viruses. Ocular findings included macular alterations (gross pigment mottling and/or chorioretinal atrophy) in fifteen eyes (75.0%), and optic nerve abnormalities (hypoplasia with double-ring sign, pallor, and/or increased cup-to-disk ratio) in nine eyes (45.0%). Conclusions: Patients presented normal anterior segment and important macular and optic nerve abnormalities. Further studies will assess the visual significance of these alterations.


RESUMO Introdução: No ano de 2015, foi identificado no Brasil um aumento da prevalência de microcefalia em vinte vezes. Esta malformação foi associada pelo Ministério da Saúde à transmissão vertical do Zika vírus (ZIKV). Método: Investigamos dez lactentes com diagnóstico clínico presumido de microcefalia relacionada à ZIKV, que apresentavam anormalidades oculares, nascidos entre maio e dezembro de 2015. Resultados: Sete mães (70,0%) relataram sintomas (mal-estar, rash e artralgia) durante a gestação, sendo seis (85,7%) no primeiro trimestre. No dia da avaliação nenhuma alteração ocular foi identificada nas mães e elas não relataram sintomas oculares durante a gestação. Sorologia foi negativa para toxoplasmose, rubéola, citomegalovírus, sífilis e vírus da imunodeficiência adquirida (HIV) em todos os lactentes. Os achados oculares incluíram alterações maculares (depósito pigmentar grosseiro e/ou atrofia coriorretiniana) em 15 olhos (75,0%) e alterações do nervo óptico (hipoplasia do disco com sinal do duplo anel, palidez e/ou aumento da escavação papilar) em nove olhos (45,0%). Conclusões: Os pacientes apresentaram segmento anterior normal e importantes e anormalidades maculares e do nervo óptico. Estudos futuros vão avaliar a importância destas alterações visuais.

ABSTRACT

Objetivo: describir los posibles efectos sobre el epitelio pigmentario retiniano generados por la primera aplicación del tratamiento multiterapéutico cubano en pacientes con retinosis pigmentaria. Métodos: se realizó una investigación descriptiva longitudinal prospectiva, donde se seleccionaron 32 pacientes con retinosis pigmentaria, a quienes se les aplicó el tratamiento multiterapéutico cubano para esta enfermedad. Se utilizó un video angiógrafo de Heidelberg tipo 2 para realizar oftalmoscopia confocal por barrido láser infrarrojo, para adquirir y procesar imágenes de la capa media de vasos coroideos, antes del tratamiento, 15 días y un año después de realizar este. El análisis de los resultados se realizó mediante Statistica 6.0 y SPSS 15.0 sobre Windows. Resultados: un año después del tratamiento se observó que 6 ojos derechos y 5 ojos izquierdos mostraron diferencias no significativas con aumento de una milimicra al comparar los resultados en los tres momentos del estudio. Al mismo tiempo 4 ojos derechos y 4 izquierdos mostraron disminución no significativas de una milimicra. El resto de los 45 ojos estudiados no modificaron los resultados durante la investigación. Conclusión: la aplicación del tratamiento multiterapéutico cubano para tratar pacientes con retinosis pigmentaria es una alternativa que conserva el epitelio pigmentario retiniano(AU)


Objective: to describe the possible effects on the retinal pigmentary epithelium after the first application of the Cuban multitherapeutic treatment for patients with retinitis pigmentosa. Methods: a prospective, longitudinal and descriptive study of 32 patients with retinitis pigmentosa, who had received the Cuban multi-therapeutic treatment for this disease. There was used Heidelberg Retinal Angiograph- 2 to perform infrared laser scanning confocal ophthalmoscopy in order to take and process images from the medial layer of the choroidal vessels before, 15 days, and one year after treatment. The results were analyzed with Statistica 6.0 and SPSS 15.0 on Windows. Results: one year after the treatment, 6 right eyes and 5 left eyes showed insignificant differences with increase of one milimicra when making a comparison of the results of the three testing moments of the study. At the same time, 4 right eyes and 4 left eyes showed insignificant decrease of one milimicra. The rest of the 45 studied eyes did not change the results of the research study. Conclusion: the Cuban multitherapeutic treatment for retinitis pigmentosa is an alternative that preserves the retinal pigmentary epithelium(AU)

ABSTRACT

A 28-year-old, non-smoker pregnant woman who was initially diagnosed to have deep vein thrombosis and pulmonary thromboembolism earlier in pregnancy, presented at 22 weeks of gestation with dyspnoea, visual loss initially in the right eye and then in the left eye. Fundoscopic examination revealed metastatic foci, suggestive of choroid metastases. Computed tomography of the chest revealed a right hilar mass. Fibreoptic bronchoscopy and bronchoscopic biopsy confirmed lung adenocarcinoma. As the patient and family wished to continue with the pregnancy, chemotherapy with cisplatin and was administered from the 31st week of pregnancy and she had undergone Caesarian section in the 32nd week and the baby was healthy. We report this case as it is probably the first reported case of lung cancer presenting with choroidal metastasis in a pregnant woman.

ABSTRACT

The purpose of this review was to analyze and describe simulation methods for practical learning and training of the ophthalmoscopy exam and to organize them into specific topics relative to each principle of operation, while evaluating their preliminary results. A critical review of articles that described and evaluated simulated models for ophthalmoscopy published in the last ten years (2004-2014) was performed. One hundred articles about ophthalmology and simulation were found in national and international periodicals, but only a few discussed the examination of the posterior pole of the eye. For this study, 25 articles were considered; those articles described simulation methods, general concepts, and its actual use in ophthalmoscopy. There were many different simulation methods described, but only few articles proved their efficacy or performed a comparison between models. Review of this topic may give information for the critical analysis of the simulation devices and ideas for the development of new ones.


O objetivo desta revisão foi analisar e descrever os métodos de simulação, apresentados em um conjunto de artigos, sobre o ensino prático e treinamento no exame de oftalmoscopia e organizá-los em tópicos específicos, referentes aos seus princípios de funcionamento, avaliando resultados preliminares. Uma revisão crítica de artigos que tratam sobre modelos de simulação para oftalmoscopia que foram criados nos últimos dez anos (2004-2014) foi realizada. Após a análise de 100 artigos, encontrados em periódicos nacionais e internacionais, sobre oftalmologia e simulação, percebemos o pouco material existente em relação ao exame de fundo de olho. Para este estudo, apenas 25 foram considerados, os quais descrevem métodos de simulação, conceitos gerais e seu uso atual na oftalmoscopia. Vários métodos de simulação já foram descritos, mas poucos artigos provam sua eficácia ou realiza uma comparação entre diferentes modelos. Uma revisão deste tópico pode contribuir para possíveis análises críticas de dispositivos de simulação já existentes e ideias para criação de outros.

A 53-year-old Asian woman was treated with hydroxychloroquine and chloroquine for lupus erythematosus. Within a few years, she noticed circle-shaped shadows in her central vision. Upon examination, the patient's visual acuity was 20 / 25 in both eyes. Humphrey visual field (HVF) testing revealed a central visual defect, and fundoscopy showed a ring-shaped area of parafoveal retinal pigment epithelium depigmentation. Fundus autofluorescence imaging showed a hypofluorescent lesion consistent with bull's eye retinopathy. Adaptive optics scanning laser ophthalmoscope (AO-SLO) revealed patch cone mosaic lesions, in which cones were missing or lost. In addition, the remaining cones consisted of asymmetrical shapes and sizes that varied in brightness. Unlike previous studies employing deformable mirrors for wavefront aberration correction, our AO-SLO approach utilized dual liquid crystal on silicon spatial light modulators. Thus, by using AO-SLO, we were able to create a photographic montage consisting of high quality images. Disrupted cone AO-SLO images were matched with visual field test results and functional deficits were associated with a precise location on the montage, which allowed correlation of histological findings with functional changes determined by HVF. We also investigated whether adaptive optics imaging was more sensitive to anatomical changes compared with spectral-domain optical coherence tomography.

ABSTRACT

OBJETIVOS: Estudar a habilidade diagnóstica do tomógrafo retiniano de Heidelberg (HRT II), GDx analisador de fibras nervosas (GDx), perimetria azul-amarelo (SWAP), tecnologia de frequência duplicada (FDT) isoladamente e em conjunto no diagnóstico do glaucoma. MÉTODOS: Sessenta glaucomatosos e 60 pacientes normais foram submetidos a exames de HRT II, GDx, SWAP e FDT. HRT foi considerado alterado quando pelo menos uma região do anel neurorretiniano esteve fora dos limites da normalidade, conforme a análise de regressão de Moorfields. GDx alterado foi definido quando pelo menos um índice foi considerado pelo programa do equipamento como fora dos limites normais, excluindo-se o índice simetria, ou ainda quando no gráfico "the deviation from normal graph" apareceu um quadrante com significância abaixo de 5%. O FDT foi considerado anormal quando pelo menos uma região testada apresentou-se com defeito severo ou com a presença de dois defeitos moderados contíguos. Para o SWAP foram adotados os critérios de anormalidade propostos por Anderson. Análise de regressão logística foi realizada. RESULTADOS: Quando foram estudadas as tecnologias isoladamente, a análise de regressão logística apresentou melhores índices de razão das chances para glaucoma com exames positivos para o HRT (22,49), seguido pelo SWAP (21,71). FDT (3,97) e GDx (2,73). Quando se associaram exames positivos de diferentes tecnologias, as razões das chances aumentaram. Nos casos com exames de HRT, FDT e SWAP fora dos limites normais, a razão das chances foi de 252,6 e com HRT, SWAP e GDx alterados, 173,1. Quando associamos exames positivos de diferentes tecnologias, a razão das chances dos pacientes serem glaucomatosos aumentou consideravelmente, chegando a 689,7 com todos os exames fora dos limites normais, o que ocorreu em 26 pacientes deste estudo. CONCLUSÕES: A análise de regressão logística confirmou que a presença de exames alterados de HRT ou SWAP apresentam as maiores razões das chances de glaucoma. A associação de exames alterados aumentou a razão das chances, principalmente, quando o HRT e o SWAP estavam fora dos limites normais.


PURPOSES: In this research was studied the diagnostic ability of the Heidelberg Retinal Tomograph (HRT II), GDx nerve fiber analyzer (GDx), Short-wavelenght Automated Perimetry (SWAP) and Frequency-doubling technology (FDT), all of them together and also separately in the diagnosis of glaucoma. METHODS: Sixty glaucomatous patients and 60 normal ones were submitted to examination with the technology of HRT II, GDx, FDT and SWAP. In the interpretation of HRT, based on the Moorfields regression analysis, were considered abnormal when at least one area of the neuroretinal ring was outside the bounds of normality. GDx was defined as altered, when it was observed that at least one index was considered by equipment program as outside of normal limits, excluding the index symmetry, or when "the deviation from normal graph" appeared a quadrant with significance below 5%. The FDT was considered abnormal when at least one region tested presented with severe defects, or two contiguous moderate defects in at least two reliable tests. For SWAP was adopted the abnormality criteria proposed by Anderson. Logistic regression analysis was performed. RESULTS: When the technologies were studied separately, the Logistic regression analysis showed better odds ratio rates to test positive for glaucoma with HRT (22.49), followed by EAP (21.71), FDT (3.97) and GDx (2.73). When positive tests were associated with different technologies, the odds ratios increased. In cases with HRT, FDT and PAA tests, outside normal bounds, the odds ratio was 252.6. Concerning the HRT, GDx and PAA the result have changed to 173.1. When we associate positive tests from different technologies, the odds ratio of glaucomatous patients have increased considerably, reaching 689.7 with all the tests outside normal bounds, which have happened with 26 patients of this study. CONCLUSIONS: The Logistic Regression analysis confirmed that the abnormal tests of HRT and PAA have shown to be the highest reasons for glaucoma occurrence. The association of abnormal tests increased the odds ratio, especially when the HRT and PAA were out of normal bounds.

ABSTRACT

Adult foveomacular vitelliform dystrophy is a rare pathology. Less than 1% of the reported cases display perifoveal capillary permeability. The three-year follow-up period of the case revealed a rare form, which had not yet been documented. The patient was a 40-year-old female with normal visual acuity, and a minor complaint of metamorphopsia on the left eye. Retinography showed a perifoveal yellowish subretinal area OS.Angiography showed perifoveal leakage OS. Follow up showed that, over 3 years, capillary incompetence disappeared and the yellow area underwent alterations, becoming atrophic OS. Angiography also showed hyperfluorescence (windows defect). Towards the end, it resembled the appearance of late stage of Best's Disease...


Distrofia viteliforme foveomacular do adulto é uma patologia rara. Menos de 1% dos casos relatados mostram permeabilidade capilar perifoveal. O acompanhamento de 3 anos deste paciente revelou uma forma rara, ainda pouco documentada. O paciente era uma mulher de 40 anos de idade com acuidade visual normal, mas com queixa de metamorfopsia leve no olho esquerdo. A retinografia mostrou uma área sub-retiniana amarelada, perifoveal, com hiperfluorescência na angiofluoresceinografia. O seguimento mostrou, em 3 anos, que a incompetência capilar desapareceu e que a área, antes amarelada, tornou-se atrófica, com hiperfluorescência na angiografia, lembrando o aspecto tardio da doença de Best...
